17,529 AI conversations later: health and wellness is not one AI market, it is five

TAMPA, Fla., Aug. 19, 2026, GOSH AI ran 17,529 AI conversations across five health and wellness categories to see who the models recommend. The answer was not one market. It was five. Pilates studios have a default brand that AI reaches for in more than 90 percent of geographies. Med spas have no national default at all, and the leader holds low single-digit share. Same recommendation layer. Five different competitive realities.

The report, The State of Health and Wellness in AI Recommendation and Discovery, is published today. It argues that the AI recommendation layer has structure. It is identifiable, it is measurable, and it changes by category. Massage and wellness studios behave like an oligopoly, with roughly three brands controlling the layer and real survival space left for independents. Chiropractors show a national brand winning aggregate share while the local default stays open in half to three quarters of markets. Nutritionists and dietitians are stranger still: AI defers to hospital systems, academic medical centers and insurance pathways over commercial brands, so the default itself is not commercial.

The practical consequence is that most brands are running the wrong play. A national campaign does not close a structural gap in a default-dominant market. In a fragmented one, a single-location operator can own the AI default in its own city. The report names five structures, assigns one category to each as evidence, and states plainly that the same playbook does not work across all of them.
